If you have lost your notice, do not know your notice reference, or the payment or appeal service does not recognise your details, complete this form and we will use the information you provide to help.
This form cannot be used to make an appeal.
Before submitting the form
If you are getting an error when trying to pay or appeal:

Check that your notice reference and vehicle registration match your notice exactly.

Check similar-looking letters and numbers, such as O and 0, I and 1, or S and 5.

If your most recent letter is from debt recovery or a legal organisation, follow the instructions in that letter.
