Appeal to the Northern Ireland Traffic Penalty Tribunal

Use this form if you have received a Notice of Rejection and want to appeal to the Northern Ireland Traffic Penalty Tribunal.

You must complete the form within 28 days of the date of issue of the Notice of Rejection.

Make a statutory declaration

Use this form if you have received a Notice of Enforcement of Charge Certificate and one of the statutory declaration grounds applies.

A statutory declaration is a sworn statement. You should read the form carefully before completing it.
